Two things:
You can certainly sign up for AOL IM without paying a cent to AOL -- it's completely free. Just download the client from AOL's web site or from some other download site like download.com, and the client will walk you through the signup process.
Don't be too surprised at the coverage map. Even major carriers like Verizon look like that -- remember that America is predominantly big, and open. Montana only has 500,000 people, and the majority of them can probably be covered with one or two transmitters. The rest? Just not practical to get to! You can cover probably 93% of the US population by putting transmitters along the major highway corridors.
